Sam is having nightmares. And then he wakes up in Toledo, Ohio for the Bloody Mary case. Sam's blaming himself for Jessica's death. Dean ripping into him for it. There's stuff Sam hasn't told Dean about it. (Which is the fact that he dreamt about it in advance.) Sam giving good advice to the almost victim at the end. Sam sees Jess when they're driving.
